Instructional Media Services realizes there is an increasing need for dynamic media presentations in the classroom. So we have placed this request form on our website to assist you with your needs.

	The IMS Program/Service Scheduling Form is designed to give the staff at IMS a better understanding of your project. It needs to be signed by a supervisor and if need be a NIC VP. Fill out this form and send it intercampus mail to IMS in BOS 214.
	The IMS Model Release Form allows IMS to use, edit and reproduce any and all photographs, audio and/or video footage taken of you, including negatives, proofs, positives, magnetic tape or digital media for any legitimate use by the college.

For equipment reservations or equipment check out please call 208.769.3429. We prefer a minimum two (2) days notice for equipment reservation.
