Financial Aid Forms 2020-2021

Fall 2020, Spring 2021, Summer 2021

Verification Forms:

  • High School Completion/Identity Statement Form
  • Order a Verification of Non-Filing with the IRS 4506-T form (sample included)
  • Order an IRS Tax Return Transcript from the IRS online
  • Order an IRS Tax Return Transcript with the IRS 4506-T EZ form (sample included)
  • Dependent Standard Verification Form
  • Independent Standard Verification Form
  • Unusual Enrollment History Form
  • COVID-19: Unable to obtain a Verification of Non-filing Statement

Appeal Forms:

All appeals will be assigned individually on a student's MyNIC Account. If a student needs to submit an appeal, the form will be found by logging into MyNIC > selecting Self Service > selecting Financial Aid  > then selecting Complete Required Documents.

Appeals Forms include: 

  • Financial Aid Appeal Extension Request Form
  • Financial Aid Broke Contract Appeal Form
  • Maximum Timeframe Appeal Form
  • Unsatisfactory Appeal Form
  • Financial Aid Unsatisfactory Appeal Professional Reference Letter Form

Change in Status Forms:

  • Petition for Independent Status Form
  • Request for Change Form
 

 FAFSA Income Modification Request:

The NIC Financial Aid Office FAFSA Income Modification Request is used to evaluate if a student's financial aid eligibility can be adjusted based on a recent decrease in income due to an extenuating circumstance.

  • If you ARE required to provide parent information on your FAFSA, complete the Dependent FAFSA Income Modification Request
  • If you ARE NOT required to provide parent information on your FAFSA, complete the Independent FAFSA Income Modification Request
